Siksika Nation members who reside in Canada can fill out the required forms for distribution online or in-person.

• Membership is encouraging members to sign up for direct deposit.
• Please include a copy of your banking information and identification with your distribution forms.
• All out of country Siksika Nation members please email distribution@siksikanation.com to make arrangements to receive your distribution.

ONLINE: Print off form, fill it out and email it to Membership with a copy of your banking information and ID to membership@siksikanation.com

 IN-PERSON: Print off, fill out forms and drop off in person at the Membership office in Siksika. Be sure to include a copy of your banking information and identification (ID) with your forms.
