Parent Receipt Forms
A Parent Receipt Form must be completed for each child in your care.
Direct Deposit
To have your distribution deposited to a personal bank account, complete a Personal Banking Form and attach a void cheque. Submit the form and void cheque in person at the Membership Office or email them to Membership@siksikanation.com.
Previously Submitted Banking Details
If you provided banking details for the last distribution, you do not need to resubmit them. However, if Parent Receipt Forms are not received, payment will be issued by manual cheque.
